Dr. Jarczewski is an out-of-network provider. After your initial appointment, follow-up appointments are scheduled for 45 minutes and the fee is $250.
Please contact your insurance company if applicable to inquire about your out-of-network benefits. We are happy to provide the appropriate receipt for you to submit to your insurance company for reimbursement. Payment is expected in full at the time of your appointment.
We require at least 48 hours notice to cancel or reschedule an appointment. Cancellations made within the 48 hours prior to your appointment will result in a cancellation fee, which is the cost of your appointment.
